Common causes: diets high in saturated fat and refined carbohydrates, insulin resistance, metabolic syndrome, type 2 diabetes, familial hypercholesterolemia, hypothyroidism, nephrotic syndrome, and obesity.
Medications: androgens, progestins, diuretics, beta blockers.
