Low DHEA-S most often reflects aging ("adrenopause") — levels peak in the mid-20s and fall steadily thereafter.
Other causes: adrenal insufficiency (Addison's), hypopituitarism or central hypoadrenalism, corticosteroid use, and chronic stress, illness, or metabolic disease (type 2 diabetes, insulin-resistant obesity).
